Questionnaire: Every evaluation requires a completed questionnaire, which can be completed by the owner or assigned to someone else within the company to complete. 

• Who will complete the company questionnaire? (You can assign it to others within the organization, but only those with platform access will see the final report results.) 
• Consider sharing that the email will be from no-reply@compliancehr.com, with a subject line of: Navigator OT Questionnaire. 
• Consider sharing the Navigator OT Questionnaire Checklist.
• The questionnaire email will indicate an expiration date by which the questionnaire needs to be completed and submitted. 
• Evaluations that are in progress will be canceled after 30 days.
